Kratom and Drug Tests: The False Positive Runs Both Ways

In a study presented at the 2025 ADLM conference, researchers reviewed 395 urine specimens that screened positive for mitragynine on a commercial immunoassay. When they ran confirmation testing by LC-MS/MS, 49 of those specimens contained no mitragynine at all. That is a 12.4% false positive rate on a test built specifically to find kratom.

The cause turned out to be methadone. Of the false positives where methadone testing was also ordered, 19 out of 23 had methadone and its metabolite EDDP present. Cheng and Chen, the study authors, spiked clean samples and confirmed it: methadone at 100 µg/mL and EDDP at 100 µg/mL both pushed the CEDIA mitragynine assay past its 50 ng/mL cutoff. The assay's own package insert had claimed methadone at that level would read negative.

The Same Interference Works in Reverse

Five years earlier, a team at the University of Virginia found the mirror image of this problem. Pierre, Gineste, and Bazydlo reported in 2020 that kratom alkaloids were causing false positive methadone screens. They spiked specimens with mitragynine and 7-hydroxymitragynine at 10,000 ng/mL and both produced indeterminate results on the Thermo Scientific CEDIA EDDP immunoassay. Their data pointed to a glucuronidated kratom metabolite as the interfering species, with absorbance rising sharply after enzymatic hydrolysis.

So a kratom user can flag as a methadone user, and a methadone patient can flag as a kratom user. Both errors trace back to the same antibody, and both disappear under mass spectrometry.

Why Antibodies Get Confused

Mitragynine is an indole alkaloid of the corynanthe type. Its formal name includes the fragment "20β-corynan-16-carboxylate," which places it in the same structural family as yohimbine and a long list of other plant alkaloids. It carries an indole core, a methyl ester, and two methoxy groups.

Immunoassays do not identify molecules. They present an antibody raised against one target and measure how strongly something in the sample binds to it. Any compound whose shape and charge distribution partially match the target can occupy the binding site. Methadone and mitragynine look nothing alike on paper, yet they apparently share enough surface features to fool a specific antibody clone. This is not sloppiness. It is the fundamental limit of the method, and it is why every regulated testing program treats a screen as presumptive and requires confirmation.

Standard Panels Versus Dedicated Kratom Panels

Kratom alkaloids are not on the SAMHSA five-panel. They are chemically unrelated to morphine, codeine, and the other opiates those assays target. That part of the common advice holds up.

What changed is that dedicated panels now exist and are ordered routinely in pain management and substance use monitoring. Labcorp's test 791750 screens urine by immunoassay at a 5.0 ng/mL threshold and confirms by LC-MS/MS at 1.0 ng/mL, reporting both mitragynine and 7-hydroxymitragynine. Quest offers comparable mitragynine monitoring. These are add-on orders, not part of a standard employment screen, but a clinic or a court can request them.

Military policy moved fast here. A Defense Department directive took effect December 31, 2025, and the Army followed with a memo signed June 23, 2026 banning kratom along with mitragynine and 7-hydroxymitragynine by name. The Navy and Coast Guard issued their own prohibitions in 2026.

Which Molecule the Lab Is Actually Looking For

Most panels report mitragynine as the primary analyte, with 7-hydroxymitragynine as a secondary marker. Kruegel and colleagues showed in 2019 that 7-hydroxymitragynine is not just a minor leaf constituent. CYP3A4 converts mitragynine into it inside the body. Raw leaf runs about 1 to 2% mitragynine by dry mass and typically under 0.05% 7-hydroxymitragynine, so most of what appears in a sample was made by the liver, not swallowed.

Half-life estimates vary more than any single blog post admits. Published human work has reported ranges from roughly 7 to 39 hours in one preparation and around 3 hours in a kratom tea preparation. That spread reflects genuine differences in product form, dose, and study design, not a settled number. Anyone quoting a precise detection window with confidence is overselling thin data.

Hair methods exist too. Meier and colleagues published a validated LC-MS/MS hair method for mitragynine and 7-hydroxymitragynine in 2020.

Isomers Are the Quiet Analytical Problem

Kratom contains speciociliatine and speciogynine, stereoisomers of mitragynine with identical mass. A mass spectrometer separates them only if the chromatography does its job first. Different labs use different columns and gradients, which is one reason quantitative results for the same sample can diverge between vendors.

There is a second-order concern that gets almost no attention. Mitragynine inhibits CYP2D6 with Ki values reported between 1.1 and 13 µM, shows mechanism-based inhibition of CYP3A4, and inhibits CYP2C19 alongside speciociliatine. That does not change a drug test directly. It can change how much of another drug is circulating when the sample is drawn, which is a different way of ending up with a result nobody expects.

Sometimes the Positive Is Real, and the Kratom Is Not the Reason

Sit with this one for a second. Kronstrand and colleagues investigated nine deaths in Sweden linked to a product sold as "Krypton," marketed as a potent kratom blend. Blood analysis found mitragynine at 0.02 to 0.18 µg/g and O-desmethyltramadol at 0.4 to 4.3 µg/g. Neither tramadol nor N-desmethyltramadol was present, meaning the active tramadol metabolite had been added directly to powdered leaf. The label said kratom. The pharmacology was a synthetic opioid.

Adulteration is an ongoing supply problem. The FDA has warned about kratom products contaminated with Salmonella and with concerning levels of heavy metals including lead and nickel. In 2025 and 2026 the agency shifted attention to concentrated and semi-synthetic 7-OH products, seizing roughly $1 million worth in December 2025 and stating that its actions target concentrated products rather than natural leaf containing trace levels.

If an unlabeled opioid is in the powder, a positive opiate screen is not a false positive. It is a correct result about a product that was not what it claimed to be.

What Reduces Your Uncertainty

Know what your product contains. A certificate of analysis from an independent lab, showing alkaloid percentages and screening for heavy metals and microbial contamination, tells you what is in the bag. Batch-level testing matters more than brand claims, because contamination happens at the batch level.

If a screen comes back positive and you dispute it, confirmation testing is the answer. LC-MS/MS identifies specific molecules rather than binding affinity, and it is what resolved every case described above. Request it, and disclose your botanical use to whoever ordered the test.
